We will be asking for your name and contact information for purposes including mentor-mentee matching and tracking data and insights through this program. As noted above, the remaining questions will be used for mentor-mentee matching, project reporting, and evaluation. Your name and contact information will not be shared in any public reports. Our program evaluators at Workforce Collective will have access to the raw data and will keep identifying information confidential. These terms apply to this application, our exit survey, and our 6-month program follow up.*
